How We are Making Care Fair for Everyone

We are dedicated to removing obstacles and making healthcare fair, inclusive, and accessible. Here’s how we do it:

Community Health Events: We regularly host and join events that connect people with important resources—from screenings to wellness workshops.
Language Assistance: Our multi-lingual staff and interpreter services help every patient understand their care and express their needs, in Cantonese, Mandarin, English and other languages.
Social Determinants of Health (SDOH) Screening: We ask about housing, food security, transportation and utility needs, interpersonal safety and social support during hospital visits and follow-up, and connect patients to local programs for help where it is needed most.
Patient Navigation: Specialty trained clinical teams work with older adults, immigrant families, and those facing system barriers, guiding them through care and connecting them with resources.
Community Partnerships: Through collaborations with agencies and health plans, we are able to extend our support outside the hospital walls—helping with insurance, mental health, and other community needs.
Care Improvement Projects: We focus most strongly on helping older women avoid unplanned hospital readmissions, using extra follow-up, social support, and teamwork to make discharge safer.
